Feedback & Concerns
Below you will find a link to our Suggestions, Compliments, Concerns and Complaints form.
This form is for submitting all types of feedback, whether that be a suggestion, a compliment, a concern or a formal complaint. The form will ask you to specify the nature of your response.
Suggestions: You may wish to propose new equipment or resources that could enhance our services, or suggest improvements to existing procedures or practices.
Compliments: You may want to inform us about aspects of our service that we are performing exceptionally well or share details of an especially positive experience you have had.
Concerns: You might have identified an issue that you believe warrants our attention, though it may not necessitate immediate action. This could involve concerns regarding the behaviour of an individual or the manner in which a particular task is performed.
Formal Complaints: You might have specific, detailed concerns regarding a service, practice, or individual's behaviour that has not been adequately resolved through informal channels. Submissions of this nature will be addressed in accordance with our Complaints Policy.
You may submit this form anonymously, however, please note that it's much easier for us to follow up on anything raised if we're able to reach out for further information. Therefore please consider including your name and contact details, should you feel comfortable to do so.
All completed forms will be reviewed by Amanda Guzinska & Alannah Crockatt in the first instance.
